Health Insurance Portability and Accountability Act (HIPAA) Definition This was a federal law placed to protect patient information. How Used in Healthcare HIPAA is used to safeguard and protect patient information and without the use of HIPAA, private patient information could be exposed.
HIPAA is critical because it could have an effect on health care workers, patients and the health care facility as well. Violation of HIPAA laws could cause a health care worker to lose their job, it could violate a patient’s rights, and it could damage the facility’s credibility. 2. Electronic medical record Definition An electronic medical record is a computerized medical record used to store patient information. How Used in Healthcare Electronic medical records are used to store patient information.
Electronic medical records can contain information about the patient’s medical history, their demographics, their general health, their past and present medical conditions, treatments received, examination results, prescribed medications, doctor’s notes, and more. Electronic medical records are used because there are many advantages versus a paper method. 3. Electronic health record Definition Is an electronic record of health-related information. Electronic health records can contain information about the patient’s medical history, their demographics, their general health, their past and present medical conditions, treatments received, examination results, prescribed medications, doctor’s notes, and more. 4. Personal health record Definition Is a personal health care record created by the patient themselves. How Used in Healthcare •A personal health record is information a patient creates for themselves.
Information my include; patient’s name, birth date, blood type, emergency contact, date of last physical, dates and results of tests and screenings, any illnesses and surgeries; along with dates , list of your medications, any allergies, any chronic diseases, and any family history of illnesses. 5. Computerized provider order entry system Definition Computerized physician order entry is the process of entering medication orders electronically instead of using paper charts. How Used in Healthcare Computerized provider order entry refers to any system in which providers input prescription orders directly into a computer system.
These systems have become common and it is used mainly because it reduces medication errors. 6. Unique patient identifier Definition The identity of an individual that consists of a set of personal characters that individual can be recognized. How Used in Healthcare Identification is the proof of one’s identity. Patient Identifier is the value assigned to an individual to facilitate positive identification of that individual for healthcare purposes. Unique Patient Identifier is the value permanently assigned to an individual for identification purposes and is unique across the entire national healthcare system.
Unique Patient Identifier is not shared with any other individual, (Appavu, 1997). Where I work, we use a patient’s name (first and last), ID card, date of birth, the last four of the sponsor’s social security number and their middle initial to identify a patient. We have to use at least 3 identifiers per patient. 7. Protected health information Definition Protected health information includes any individually identifiable health information. How Used in Healthcare A disclosure of Protected Health Information is the sharing of that patient health information outside of a covered entity.
The sharing of patient health information outside of the health care component or affiliated covered entity is a disclosure. In general, any disclosure of personal health information requires an accounting at the request of the individual, unless the individual gave permission for the disclosure by signing an authorization form, (“Hipaa privacy rule:” 2003). 8. Centers for Medicare & Medicaid Services Definition The Centers for Medicare & Medicaid Services is an agency that is responsible for administration of several key federal health care programs.
How Used in Healthcare In addition to Medicare; the federal health insurance program for seniors and Medicaid; the federal needs-based program, CMS oversees the Children’s Health Insurance Program the Health Insurance Portability and Accountability Act and the Clinical Laboratory Improvement Amendments among other services, (“Centers for medicare,” 2010). 9. Covered entities Definition Cover three basic groups of individual or corporate entities: health plans, health care providers, and health care clearinghouses. How Used in Healthcare
Covered entities cover three groups in healthcare; the health plan, the provider and the health care clearinghouses. The health plan means any individual or group plan that provides, or pays the cost of, medical care. The health care provider means a provider of medical or health services. Health care clearinghouse means a public or private entity, (“Covered entity (hipaa),” 2005). 10. Health information exchange Definition Health information exchange is the mobilization of healthcare information electronically across organizations within a region, community or hospital system.
How Used in Healthcare Health information exchange provides the capability to electronically move clinical information from one health care organization to another.
